Unwanted order surcharge appearing in all my orders

Hi,

Every order has 'Line Item Example 2 Order Surcharge' of $5 added and I don't know how to get rid of this or where it has come from. I've tried looking at the rules debug output but can't see if there is a rule set up. Any one got any ideas?

It seems that the module 'Line Item Example' was responsible. The unwanted line was removed when I uninstalled the module!
